1. Acknowledgment of Risk

I, , acknowledge that training in Brazilian Jiu-Jitsu and other martial arts at Titanium Jiu Jitsu involves inherent risks, including but not limited to:

• Physical injuries such as sprains, fractures, dislocations, or concussions.
• The risk of falling, being thrown, or choked.
• Training with partners of varying skill levels, increasing the risk of injury.
• The potential transmission of communicable diseases, including but not limited to COVID-19.

I voluntarily assume all risks associated with participating in Titanium Jiu Jitsu programs and classes.

2. Release of Liability

I hereby release, discharge, and hold harmless Titanium Jiu Jitsu, its owners, instructors, employees, and any affiliated parties from any claims, demands, liabilities, or damages arising from:

• Personal injury, illness, or death.
• Damage or loss of personal property.
• Any other losses incurred while participating in training or related activities.

I understand that this waiver is binding and applies to me, my heirs, executors, administrators, and legal representatives.

3. Medical Authorization

In case of injury, I authorize Titanium Jiu Jitsu staff or emergency personnel to seek and provide necessary medical treatment on my behalf. I acknowledge that I am responsible for any medical costs incurred.

4. Equipment Liability & Hygiene Policy

I agree to:

• Properly use and maintain any gym-provided equipment.
• Replace or cover the cost of damaged or lost gym property if caused by negligence.
• Follow all hygiene policies, including wearing clean training gear and maintaining personal hygiene.
• Notify Titanium Jiu Jitsu if I am experiencing any illness symptoms before attending classes.

Failure to comply may result in suspension or termination of my membership without a refund.

5. Code of Conduct & Safety Agreement

I agree to:

• Follow all gym rules and safety guidelines.
• Respect instructors, training partners, and staff.
• Refrain from any unsportsmanlike conduct, dangerous behavior, or intentional harm.

I understand that failure to comply may result in immediate dismissal from the gym.

6. Waiver of Lawsuits & Indemnification Clause

I agree that I will not sue, file a claim, or hold Titanium Jiu Jitsu responsible for any injury, damage, or loss.

If I bring a claim or lawsuit, I agree to indemnify Titanium Jiu Jitsu for all legal costs, damages, and expenses incurred.

7. Photo/Video Opt-Out (Optional)

I do not grant Titanium Jiu Jitsu permission to use photos/videos of me for promotional or marketing purposes.

8. Governing Law & Jurisdiction

This waiver shall be governed and interpreted in accordance with the laws of [your state]. Any legal disputes shall be handled exclusively in the courts of [your county/state].
